St Luke’s HospiceSt Luke’s Hospice is a charity that cares for people approaching the end of their life in Harrow and Brent. They aim to give the people they care for the best possible quality of life, each and every day. Supporting local people and their families is at the heart of what they do – focusing on what matters the most to them is at the centre of their care. Their inpatient unit (IPU) at Kenton Grange cares for people in their final days, providing intensive care to help get symptoms under control or give respite care. However, many of the people who are cared for in the IPU go home again. Their specialist community team of nurses helps people to manage more complex needs whilst the Hospice at Home team supports people to remain in their own homes by providing practical care and support.

CAAP (Cornwall Accessible Activities Programme)CAAP is a small charity based in Cornwall formed by a group of parents of children with additional needs to support families to access activities during school holidays.Every child or young person registered with CAAP has a disability or additional need that impacts on their ability to attend mainstream holiday activities. They typically cannot play in their own communities because of safety issues and often spend school holidays indoors or in front of screens. School holidays are a time of great stress for families with disabled children with the lack of structure and this also impacts on siblings.

Rainbow HubRainbow Hub is a special charity that exists to improve the lives of children and young people with mental and physical challenges, and families, from across the North West, by providing specialist educational and therapeutic approaches to learning. As part of this they have created a special needs nursery which caters specially for early years aged children with diagnosed and emerging special educational needs.

Stepping OutRunning since 2015, Stepping Out offers group exercise and well-being schemes, including yoga and dancing, which aim to improve strength, flexibility, balance, overall fitness and general well-being as well as being part of a supportive and friendly group.

Fox & FriendsFox and Friends was set up with the aim to create a range of services which were accessible to everyone to support people who wanted to come together to learn, connect and thrive following the COVID lockdown. This includes Interactive Story Sessions, ASN Family Support Group, Clubs & Activities for Adults as well as events for communities based across the Highlands.

Ty HapusTy Hapus is Welsh for ‘Happy House’ and has provided home-from-home support and respite for people living with dementia and their families for over 11 years. The charity is primarily a service for younger people with a diagnosis under the age of 65 with activities and sessions organised within the heart of the community using local businesses, attractions and areas of interest.

Stoke & Staffodshire Downs Syndrome Social GroupStoke & Staffordshire Downs Syndrome Social Group was set up in 2016 by parents and husband and wife Julia and Michael Boardman. The aim of the group was to offer children like their son Max a safe, fun and sociable place to meet with other children who have Downs Syndrome and to create a “family” for the future where parents could support each other due to the lack of support locally for our children.
